The Absolute Elder Care Model
A commitment to ongoing, personal communication is the foundation of the Absolute Elder Care model. We work with each facility to develop a visitation schedule that reflects the specific needs and concerns of its residents. We then assign specific clinician team to each facility to ensure a greater familiarity with residents’ chronic and acute health issues as well as maintain continuity of care. Our patients’ care is our number one concern. Here are a few of the benefits of working with us:
Focused clinicians. Our physicians, nurse practitioners, physician assistants, and medical assistants work primarily in Assisted Living Facilities. This allows them to have a deeper understanding of the issues that may arise which are unique to ALF’s and their residents. They are familiar with the policies and procedures which governs ALF’s. Many of our clinicians have participated Core training sessions in order to better aid the facilities they service.
Frequent, consistent care. Team members typically work in each facility weekly, allowing them to respond promptly to the concerns of families and nursing staff.
Improved communication. Regular on-site visits help ensure concerns are promptly addressed with nursing staff, and residents, and family members, lessening the time burden and frequency of phone calls.

For residents and family members, we have a team of clinical office assistant standing by to help make sure we are available to handle any issue that may arise such as medication refills, issues with outside medical services, ordering of medical equipment, long term care paperwork, clinical paperwork for veterans benefits, and much more. For after-hours issues that can’t wait till the morning, our answering service will get you in touch with our on call provider day or night.
For non urgent issues, you will be given access to our Eclinical Works Patient Portal. Through this portal you will be able to send messages directly to our clinical staff as well as give you access to your basic lab results and general comments from your provider.
For Community staff, we provide on-call telephone support 24 hours a day with the use of an after-hours phone line which, which directly reaches one of our clinicians. If needed orders can then be given over the phone or faxed to your facility within minutes!
